The Statement Of Financial Affairs lists 25 questions that asks things like your total income for the past 3 years, any amounts you have paid to creditors, any lawsuits you are involved in, any insurance losses you have incurred, property you own that is in the possession of others, previous spouses, old addresses, any expenses you have incurred to file bankruptcy and more. Questions 19-25 are questions that pertain to individuals that own any part of a business.

WHAT YOU NEED: Financial and legal records from the past 3 years including tax returns. Any business records for companies you own any part of.

INSTRUCTIONS INCLUDED ON THE FORM: This statement is to be completed by every debtor. Spouses filing a joint petition may file a single statement on which the information for both spouses is combined. An individual debtor engaged in business as a sole proprietor, partner, family farmer, or self-employed, professional, should provide the information requested on this statement concerning all such activities as well as the individual's personal affairs.
